Fees
All fees are payable in full by the patient or parent guardian at the time the service is provided unless specific specific arrangements have been made prior to the beginning of treatment. You will be provided an itemized fee estimate prior to treatment if you and your insurance carrier require this information. An additional charge may apply for late payment of fees.
Dental insurance
The doctor will complete insurance forms relative to your treatment. However, WE DO NOT ACCEPT ASSIGNMENT. This simply means that your insurance check will go directly to you not our office. Our professional services are provided to you that the insurance company therefore you are directly responsible for payment of fees. You will be reimbursed by your insurance company according to your plan. If your insurance does not completely cover the fees for a certain procedure is usually due to one of the following reasons:
1. The procedure is not covered under your plan
2. Your plan only covers a percentage of that procedure
3. Your plan is not covering current specialist fees
4. Your annual maximum for the year has been met
If you are required to use a special insurance form please provide it to us. Alternatively, if you provide us with your insurance information, we will be happy to submit the codes and fees electronically on your behalf. This will expedite your reimbursement. Full payment is still required at the time of visit.
OHIP
Office procedures are never an OHIP benefit. The Ontario Health Insurance Plan pays for certain oral and maxillofacial surgery procedures provided you are admitted to a hospital on an inpatient or outpatient basis. Or, when certain specific procedures are performed in the hospital emergency or outpatient clinic. An OHIP covered procedure includes a surgical services while in the hospital and one visit after discharge.
However even in the hospital not all procedures or a benefit of OHIP. There may be aspects of your care in the hospital and during the pre- and post-operative which are not OHIP benefit and therefore considered non-insured benefit services (NIBS). The doctor will whenever possible identify the non-insured services prior to your admission and will discuss the fees associated with them. You are responsible for payment in full of all non-OHIP insured sevices. Your dental plan may partially reimbursed you for some of these fees. Any portion not covered by your dental insurance or by OHIP may be claimed on your income tax return. If you were given an anesthetic while in hospital and may or may not be covered by OHIP. Please discuss this with our office staff at the time of hospital booking.
For your convenience, we accept Visa, MasterCard, and Debit. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance.
